FULL COMPREHENSIVE ASSESSMENT
We assess medical needs and history, ensuring continuity of care and identifying gaps in the medical care plan. Depending on the urgency of the current situation, a full comprehensive assessment might be performed as quickly as within a few hours or days. However, assessment is always ongoing. Our care managers are constantly reviewing your medical and psycho-social needs and making recommendations as change occurs. |
